We are seeking dedicated and skilled Registered Nurses to join our Night, Evening, & Weekend Team (NEWT) which services Charlottesville, Virginia, and surrounding counties.
As a nurse in our hospice program, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients.
Responsibilities:
Consistently respond to answering service and provide home visit to assess patients' needs and provide nursing care.
Assess and re-evaluate patient needs, consulting with physicians, reporting changes, and communicating with others when a change in the Plan of Care is necessitated to ensure optimal comfort and quality of life.
Provides education to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
Provide services requiring specialized nursing skills in accordance with agency guidelines and practice appropriate documentation in a timely manner.
Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions by practicing appropriate documentation at the bedside using the Electronic Medical Record (EMR) and prepares clinical progress notes on nursing services delivered in a timely manner.
Participate in interdisciplinary team meetings, providing valuable input and contributing to the overall care plan.
Stay current with best practices, research, and developments in hospice care, seeking continuous professional development opportunities.

Qualifications:
Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
Minimum of 2 years of nursing experience preferred, preferably in hospice, palliative care, or a related field.
Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
Highly organized, self-motivated with the ability to articulate information with a diverse population, and proficient in using electronic medical records (EMR) and other healthcare software.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
