The Registered Nurse (GAP) Charlottesville CBOC position will fill in for nurses due to retire in the next several months as well as pending and/or unexpected resignations. The incumbent will provide Primary Care to Veterans through a newly developed Patient Aligned Care Team model (PACT). The PACT model allows time and structure to attain comprehensive patient centered nursing care. The team includes the Primary Care Provider, RN Care Manager, LPN, and Medical Support Assistant. The mission of the team broadens for inclusion of the patient's family, internal and community-based services that are involved in providing care to the patient. Duties include but are not limited to the following:
-Utilizes the nursing process and evidence-based practice to collaborate with the PACT. -Practices specialized skills in patient assessment via telephone and direct contact with patient in order to facilitate care transition internally as well as externally to meet the patients' need. -Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's total healthcare goals. -Performs other directly related clinical and administrative duties as assigned. Monday-Friday 8am-4:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
A minimum of 5yrs of full-time clinical experience as an RN in medical/surgical setting or combination of medical/surgical and ambulatory care setting highly desirable. Primary Care nursing experience strongly preferred. A bachelor's degree in nursing highly desirable. Experience as a Clinical Case Manager beneficial. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements:
Includes lifting, prolonged standing, frequent bending, and stooping, walking, reaching, and repetitive motion. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing, keyboarding, reading, and logical thinking. BLS/ACLS Requirements:
Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification in order to avoid any delays in the hiring process. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
