Registered Nurse (GAP) Charlottesville CBOC
The Registered Nurse (GAP) Charlottesville CBOC position will fill in for nurses due to retire in the next several months as well as pending and/or unexpected resignations. The incumbent will provide Primary Care to Veterans through a newly developed Patient Aligned Care Team model (PACT). The PACT model allows time and structure to attain comprehensive patient centered nursing care. The team includes the Primary Care Provider, RN Care Manager, LPN, and Medical Support Assistant. The mission of the team broadens for inclusion of the patient's family, internal and community-based services that are involved in providing care to the patient. Duties include but are not limited to the following:
-Utilizes the nursing process and evidence-based practice to collaborate with the PACT. -Practices specialized skills in patient assessment via telephone and direct contact with patient in order to facilitate care transition internally as well as externally to meet the patients' need. -Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's total healthcare goals. -Performs other directly related clinical and administrative duties as assigned. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
A minimum of 5yrs of full-time clinical experience as an RN in medical/surgical setting or combination of medical/surgical and ambulatory care setting highly desirable. Primary Care nursing experience strongly preferred. A bachelor's degree in nursing highly desirable. Experience as a Clinical Case Manager beneficial. Grade Determinations:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification in order to avoid any delays in the hiring process. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social).
